CLER: Patient Safety Pathway
This page focuses on the “Patient Safety Pathway” of the CLER initiative.
What: Patient Safety is a System-Based issue that causes or allows something to happen to a patient that would not be right if you or one of your family members was the patient. It is everyone’s responsibility to enter Occurrence or MIDAS reports so that the institution can address these issues and make it better and safer. PATIENT SAFETY = MIDAS REPORTS (which make us all function better as a system / team).
Why: Not only is it good for our patients (which is what it is all about), it is required by the ACGME that faculty and residents make occurrence reports. We report data every other year during CLER (Clinical Learning Environment Review) visits as to how many reports were made by each.
Traditionally, physicians were trained to “not report” and this lack of a Just Culture is working against us and against our common goal of improvement. National trends show that residents, in particular, are not reporting enough. Some institutions have gone so far as to mandate that everyone submit 2 occurrence reports per year in order to increase the numbers; that’s something for us to think about in terms of changing the culture that persists here at KDCHD.
How: Making an Occurrence or MIDAS report is easy! Go to KDNet on any computer. You will find “MIDAS Occurrence report” listed on the lefthand side. Answer the questions and complete the report. If you would like to remain anonymous in making your report, please merely add that you are GME resident in the optional field. Some physicians have expressed concerns over the time it takes to enter a report and so the Medical Staff Office has set up a hotline ext. 6889 that allows you to speak your issues into a voice recording and they can enter the information on your behalf!
